Kim Phan, M.B.A.

Founder, Chief Executive Officer

Kim Phan has over 20 years of experience in managed care, and her post-acute care background supports her strategic guidance in helping Care Connect grow and succeed. Prior to embarking on this development, she served as General Manager for CareMore Touch, a Health Essentials sister company. Kim received her Master of Business Administration and her Bachelor of Arts in Economics from University of California, Irvine. Additionally, she served as Associate Administrator for Orange Coast Memorial Medical Center, Associate Administrator at FHP Hospital, and Administrator of FHP's 99-bed Skilled Nursing Facility.

George Fields III, D.O.

Chief Medical Officer

George Fields is a Board Certified Family Physician who has been working as a Chief Medical Director for Gerinet Healthcare since 2016. Previously he worked in Cerritos with CareMore Health Pla. In his role at CareMore he was directly responsible for the Institutional Special Needs Plan, Duals Initiative and Palliative Care Programs. He also served as a Senior Medical Officer of CareMore, in charge of our Iowa, Ohio, Virginia and Tennessee markets since 2006. Dr. Fields has a keen focus on the redesign of health care and a desire to deliver better care to every patient. In 2015 he was awarded Lifetime Achievement at SeniorServ's Annual Senior Care Hero Awards. Dr. Fields graduated from the Western Univ of Health Sciences College of Osteopathic Medicine of the Pacific in 1983.

Len Whyte.

Chief Financial and Administrative Officer

Len Whyte is Chief Financial and Administrative Officer for CareConnect Health Services, Inc. He has over 25 years of experience in managed healthcare. Most notably, he spent 17 years with PacifiCare Health Systems in various financial and operations positions. He was part of the initial team tasked with creating and building PacifiCare's inhouse behavioral health division where he spent ten years as its Chief Operating Officer. He also spent nine years with Centene Corporation as Chief Operating Officer of Centene's inhouse behavioral health division. He oversaw the growth from $200 million serving six states to $1.2 billion serving eighteen states. Len has an MBA from the Kellogg School of Management at Northwestern University and a BA from St. Olaf College. Len enjoys, traveling, learning to golf and volunteering in his Church's assisted living ministry.
